How do I get my child into Leaps and Bounds?

1. Call your child’s pediatrician or primary care physician for a script for OT, PT, or speech therapy (needs to come from a PA, NP, or MD)

2. Once the referral is received by Leaps and Bounds, you will receive paperwork in the mail to complete and return

3. Once the paperwork is returned, you will be placed on the wait list

4. You will be called by the office to schedule an appointment when you reach the top of the wait list
